- Exposure to armed conflicts profoundly impacts individuals and their communities but provides an opportunity to experience posttraumatic growth (PTG). Whereas most studies examined the direct exposure to the traumatic event, this study investigated the direct and indirect links between type of war exposure (self-exposure, close others' exposure, acquaintances' exposure), and rumination (deliberate and intrusive) to PTG in Israelis following the Israel-Hamas war. Participants (n = 455) completed measures of war exposure, rumination, and PTG. Exposures of close-others and acquaintances were associated with higher PTG, partly through increased deliberate rumination. The theoretical significance lays in understanding that exposure to trauma through others in one's community may be more strongly linked with PTG than previously acknowledged, and highlighting the positive role of deliberate rumination. Attention to those who have been exposed to traumatic events indirectly, and interventions that encourage deliberate rumination, are essential for empowering individuals within their communities. - Source: PubMed

Endoscopic mucosal resection (EMR). endoscopic submucosal dissection (ESD) and other related endoscopic resection techniques can provide curative-intent treatment with accurate histopathologic staging, but their safe and reproducible delivery depends on robust service design rather than technical expertise alone. This narrative review synthesizes practical insights from Western institutions on the development and operation of a high-quality endoscopic resection unit. Key domains include service model and referral pathways, patient selection and peri-procedural planning, procedural environment, equipment and accessories, multidisciplinary support, complication rescue pathways, workforce training, quality monitoring, and institutional sustainability. Particular emphasis is placed on standardization, multidisciplinary integration, prospective audit, and governance structures that support consistent outcomes and long-term program viability. High-quality resection units should be regarded not simply as procedural services, but as organized clinical platforms that deliver safe, scalable, and organ-preserving treatment, with resilience built within the team framework. - Source: PubMed

Hsieh Yi-HsuanLin Chi-Kang - Little is known about what factors contribute to posttraumatic stress disorder among military healthcare personnel during a deployment. Accordingly, the purpose of this study was to explore the association among demographic variables, psychological and occupational stressors, and protective resources with probable PTSD caseness and PTSD symptom severity in a sample of 1161 military healthcare personnel who were deployed to a combat hospital in Balad, Iraq, between 2004 and 2011. In this cross-sectional observational study, participants completed anonymous self-report measures while in theater. Results indicated that a model with reports of depression symptom and negative affect severity, and exposure to combat and futility stressors (i.e. stressors that service members do not feel fully prepared to manage) was associated with probable PTSD caseness (χ2 (17, = 739) = 236.62, < .001). The model explained approximately 63.6% of the variance in probable PTSD caseness. A hierarchical linear regression predicted PTSD symptom severity, adding gender and trainable stressors (i.e. stressors that service members feel fully prepared to manage) as independent variables to the variables above ( (17, 839) = 89.76, < .001; adjusted = .64). In deployed military healthcare personnel, depression and negative affect (e.g. dysphoria, anhedonia, anger) may be behavioral proxies for PTSD severity and when present with noteworthy exposure to combat and healthcare stressors may provide discernable information that peers and leaders can use to check-in to see what service members may need to recover confidence and competence, if applicable. - Source: PubMed
