- Most Mendelian randomization (MR) of dietary intake use the full set of genome-wide significant (GWS) variants in the instrumental variable (IV), likely biasing causal estimates due to pleiotropy. To characterize the common methods to handle pleiotropy in dietary intake MR, we conducted a scoping review of the literature on dietary intake MR studies. We extracted information on IV construction, assessment of pleiotropy, and sensitivity analyzes revealing that only 20% of studies used an IV with functional plausibility. In the absence of functionally-informed IVs, we tested if two-sample MR using GWS variants filtered for pleiotropic associations through phenome-wide association studies (PheWAS) could identify diet-health relationships supported by existing nutrition science, focusing on oily fish and alcohol intake, the latter of which has a functionally-informed IV for comparison (rs1229984 in the ADH1B gene). To further explore this question, we performed multivariable MR and employed MR-CAUSE. The numerous models consistently supported that oily fish reduced triglycerides. In contrast, GWS and PheWAS-filtered IVs suggested that alcohol decreased alanine aminotransferase levels, whereas the functional IV (rs1229984) found the opposite expected relationship. Isolating the direct effect of dietary intake from GWS IV remains challenging. Future work should focus on identifying functional variants impacting dietary behavior. - Source: PubMed
Sutton Kristen JGervis Julie EJatoi MoomalHwang Liang-DarHendricks Audrey EGhosh DebashisWesterman Kenneth ECole Joanne B - Microcystin-LR (MC-LR), a hepatotoxic aquatic contaminant classified by IARC as a Group 2B carcinogen, was investigated using an integrated network toxicology and machine learning framework to explore potential molecular mechanisms underlying MC-LR-related hepatocarcinogenesis. By integrating GEO transcriptomic datasets, differential expression analysis, WGCNA, and predicted MC-LR targets, we identified 24 candidate genes potentially associated with MC-LR-related HCC. Functional enrichment analyses implicated these genes mainly in complement and coagulation cascades and metabolic processes. Machine learning consensus analysis prioritized five core genes-AKR1C3, FABP5, CA2, ADH1B, and EPHX2-each showing an AUC greater than 0.79 and suggesting moderate discriminatory potential. Single-gene GSEA linked these genes mainly to ribosome-related pathways, while single-cell transcriptomic analysis revealed cell-type-specific expression heterogeneity within the HCC microenvironment. Molecular docking provided exploratory structural evidence for possible in silico compatibility between MC-LR and the encoded proteins. However, further experimental validation is required to confirm this hypothesis. Overall, these findings establish a multidimensional framework for investigating MC-LR-associated hepatotoxicity and nominate candidate molecular nodes for future biomarker assessment, functional validation, and environment-related HCC risk evaluation. - Source: PubMed

This review proposes a Japan-oriented framework for translating the diagnosis of alcohol-related diseases into behavioral change interventions. Central to this framework is the concept of the "teachable moment," in which a new diagnosis increases patient motivation and creates opportunities for timely clinical intervention. We reviewed common alcohol-related conditions, including liver disease, alcohol-related cancers, acute pancreatitis, atrial fibrillation, hypertension, hypertriglyceridemia, gout, sleep disorders, and pneumonia, and summarized the evidence that abstinence or reduced alcohol consumption can improve outcomes. We also present an integrated approach that combines brief intervention, liver function-guided pharmacotherapy, nutritional rehabilitation, and specialist collaboration.
