- Chemical, biological, radiological, and nuclear (CBRN) incidents present escalating risks across the Middle East and North Africa (MENA) amid geopolitical instability, cross-border threats, and evolving non-state actor capabilities. This policy analysis examines why prevailing case-level and ministry-siloed governance is insufficient for population-level CBRN readiness, weighs alternative coordination models, and outlines an artificial intelligence (AI)-enabled, centrally coordinated public-health strategy adapted to the region's heterogeneity and resource constraints. We propose a National Emergency Management Advisory Council to provide statutory inter-ministerial authority and stewardship for a National CBRN Dashboard that delivers decision support, inventory tracking, simulation, and rapid triage. We situate this within existing backgrounds and analyze legal authority, financing, data governance, and feasibility in low-resource settings. While prototype AI models report high accuracy for antidote optimization, agent classification, and triage, we argue these metrics reflect controlled research, not operational readiness, and require external validation, robustness testing, and cybersecurity safeguards. Pre-exposure prophylaxis (PrEP) is a proven and effective prevention strategy, but challenges in access, equity, adherence and persistence limit its population impact. Our aim was to identify current barriers to PrEP adoption in Spain and develop actionable solutions. We conducted a structured consensus process (September 2024-May 2025) with 19 experts from fields including infectious diseases/HIV care, sexual health/sexually transmitted infection services, primary care, emergency medicine, nursing, pediatrics, hospital pharmacy, public health, epidemiology/biostatistics, and community-based organizations. It included semi-structured interviews, multidisciplinary small-group sessions, and two plenary workshops. Complementary descriptive surveys of healthcare professionals (HCPs) and NGO/community health centers ( = 150) and current/potential/past PrEP users ( = 95) provided contextual data. Consensus statements were developed using a facilitator-led approach based on nominal group techniques, with community representatives contributing lived-experience insights. The panel identified key challenges, including fragmented access (hospital-centered), capacity constraints, restrictive eligibility criteria, knowledge gaps, adherence and persistence issues, and limitations in information systems. To address these, a focused decalogue was developed to promote decentralization, equity, person-centered care and preparedness for transformative innovation in PrEP. The ComPrEmetidos consensus offers an operational roadmap to move Spain from policy availability to program performance and more effectively reduce new HIV infections in Spain. Long-acting pre-exposure prophylaxis (PrEP) formulations, such as bimonthly cabotegravir (CAB) and biannual lenacapavir (LEN) injections, may be particularly effective for HIV prevention in this population. We conducted interviews with 17 individuals in the western US who provided a service in the past month to an HIV-negative man who has sex with men and uses methamphetamine. Interviews were audio recorded and transcribed. Interviews inquired about barriers to each step of the PrEP cascade and strategies to overcome them. Transcriptions were coded by at least two analysts and reconciled. Interviews revealed several barriers to long-acting PrEP engagement among MSM who use methamphetamine across the cascade, most notably homelessness, which included competing priorities and inconsistent phone access. Despite the identification of housing instability as a barrier to long-acting PrEP engagement, it was also cited as a reason why long-acting PrEP may be particularly promising for MSM who use methamphetamine. Anticipated stigma was also identified as a primary barrier to long-acting PrEP engagement. Provider-level barriers were described, including overburdened clinics, limited hours, requisite staffing expertise, and billing restrictions. Strategies to increase long-acting PrEP provision among this population who could benefit from PrEP included reducing provider-side barriers and integrating long-acting PrEP with other appealing services (e.g., syringe services programs), extended hours, mobile delivery, and contingency management. - Source: PubMed
