Although it was extensively used in many areas, studies on its polysaccharide fractions were still limited at present. In this study, ultrasonic-assisted extraction was adopted to prepare Allium macrostemon Bunge polysaccharides (AMBP), and three strategies including Box-Behnken design (BBD), genetic algorithm optimized back propagation neural network (GA-BP), and gradient boosting decision tree (GBDT) were separately employed to optimize the extraction parameters. Experimental results demonstrated that the GA-BP model exhibited superior data fitting and prediction performance with remarkably higher prediction accuracy compared with the BBD and GBDT models. The coefficient of determination (R) reached 0.9829, with a RMSE of 0.3086 and a MAE of 0.1717. The GA-BP model yielded the following predicted optimal conditions: temperature 50 °C, extraction time 33 min, and liquid-to-solid ratio 1:30 g/mL. The neutral polysaccharide AMBP-1 was isolated and purified by column chromatography, and its structural characterization was subsequently performed. The results indicated that AMBP-1 was a homogeneous polysaccharide with a relative molecular mass of 5.56 kDa, exhibiting an irregular sheet-like morphology, and its backbone structure was → 4)-β-D-Glcp-(1 → 6)-β-D-Galp-(1 → . Preliminary in vitro cellular assays and in vivo immunosuppressive mouse models demonstrated that AMBP-1 possessed immunomodulatory activity, and multi-omics analysis revealed potential correlations between its effects and alterations in gut microbiota, serum metabolites, and immune-related transcriptional profiles. In conclusion, this study established an efficient ultrasonic-assisted extraction process optimized by the GA-BP model, providing a reliable technical foundation for the preparation of AMBP, and laid the groundwork for the application of AMBP-1 in functional foods and nutraceuticals. - Source: PubMed

Data from adults in Kinshasa are particularly scarce. We conducted a cross-sectional study between March 2023 and April 2024, enrolling 279 clinically stable adult SCD patients from 14 centers in Kinshasa. Tubulopathy was defined by the concomitant presence of three abnormalities: urinary α1-microglobulin-to-creatinine ratio (A1M/Cr) ≥20 mg/g, dipstick glucosuria ≥1+ with plasma glucose <126 mg/dL, and urine pH ≥5.5. Glomerular involvement was assessed by urinary albumin to creatinine ratio (UACR ≥30 mg/g). Measured GFR (mGFR) was obtained by iohexol clearance. Multivariable logistic regression was used to identify independent factors associated with tubulopathy. Tubulopathy was identified in 58 patients (20.8%). Compared with those without tubulopathy, affected patients had significantly higher median A1M/Cr (44.7 vs. 14.2 mg/g; < 0.001) and UACR (22.1 vs. 10.7 mg/g; = 0.007). They also displayed lower mean eGFR by CKD-EPI (107.4 vs. 115.9 mL/min/1.73 m²; = 0.031) and higher LDH levels (217.0 vs. 209.1 IU/L; = 0.016). In multivariable analysis, independent predictors of tubulopathy included higher LDH (per 100 IU/L increase) (aOR 2.53, 95% CI 1.14-5.61), UACR ≥30 mg/g (aOR 1.94, 95% CI 1.18-5.18), while hydroxyurea use was independently associated with lower odds of tubulopathy (aOR 0.37, 95% CI 0.16-0.87). One in five stable adult SCD patients in Kinshasa presented with tubulopathy. This phenotype was associated with hemolysis, albuminuria, and lower eGFR, whereas hydroxyurea use was inversely associated with tubulopathy. - Source: PubMed

This study quantified urinary 8-hydroxy-2'-deoxyguanosine (8-OHdG) and 8-isoprostane as markers of oxidative damage, and plasma α-microglobulin (A1M) as an endogenous antioxidant, while exploring the influence of pre- and intraoperative factors. : In a prospective cohort of 40 term neonates with critical congenital heart defects undergoing open-heart surgery, serial urinary and plasma samples were collected perioperatively. Biomarker concentrations were analyzed using mixed-effects regression models to assess associations with postnatal age, sex, preoperative oxygenation, oxygen surge at bypass initiation, and cell-free hemoglobin in the prime solution. : Urinary 8-OHdG and 8-isoprostane increased following bypass separation; 8-OHdG remained elevated through postoperative days 0-2, while 8-isoprostane returned toward baseline by day 1. Plasma A1M declined at bypass initiation, recovered to preoperative levels at separation, and rose thereafter. Females exhibited higher A1M concentrations throughout. Longer time to surgery was associated with greater preoperative oxidative stress, and lower preoperative arterial pO correlated with increased 8-isoprostane at bypass separation. No statistically significant associations were identified between intraoperative variables and biomarker levels, although the study was designed to be a hypothesis generating study and not powered to detect modest intraoperative effects. This exploratory study delineates distinct perioperative trajectories of oxidative stress and antioxidant response in neonates undergoing open-heart surgery, with surgical timing, preoperative hypoxemia, and sex emerging as relevant associations. No statistically significant associations were identified between intraoperative variables and biomarker levels; however, the study was hypothesis-generating in design and not powered to detect modest intraoperative effects. - Source: PubMed
