- Primary adenoid cystic carcinoma (ACC) of the thymus is extremely rare. Although thymic ACC is generally slow growing, complete surgical resection is essential due to its tendency for local invasion. Here, we report a case of primary thymic ACC successfully treated by complete resection using a single-port subxiphoid thoracoscopic approach. A 63-year-old asymptomatic male patient was referred after an anterior mediastinal mass was incidentally detected during a routine health screening. Imaging revealed a small, well-circumscribed tumor without evidence of invasion. At 2.2 cm, the lesion was among the smallest thymic ACCs reported to date. Thymoma was suspected and single-port subxiphoid thoracoscopic thymectomy was performed. The patient's postoperative course was uneventful. The histopathological and immunohistochemical findings were consistent with ACC, and no other primary lesion was identified. Complete resection was achieved with minimally invasive thymectomy. This case illustrates that thymic ACC may rarely present as a small, early-stage tumor indistinguishable from thymoma, and it highlights the importance of complete resection with negative margins and long-term surveillance given the potential for delayed recurrence. - Source: PubMed

We report seven cases of metastatic carcinomas to the liver with acinar cell differentiation (two acinar cell carcinomas, four carcinomas with mixed acinar and neuroendocrine differentiation, one mixed acinar ductal carcinoma) that lacked many of these features, and, as a result, were misclassified. The cases were prospectively identified in our routine confirming consultation service. Clinical data and histology were retrospectively reviewed. The seven patients ranged in age from 48 to 72 years with a male predominance (5 males, 2 females). Five patients had a known pancreatic mass, and all patients had multiple liver lesions on initial imaging, prompting liver biopsies. Three of seven liver tumors were diagnosed at the submitting institution as high-grade neuroendocrine tumors (NETs) as they all expressed synaptophysin and/or chromogranin, had a high Ki67 proliferation index, and lacked cytoplasmic granules or acinar architecture. The remaining four liver tumors were diagnosed as adenocarcinoma as they lacked prominent nucleoli, had prominent fibrotic stroma (3 cases), lacked eosinophilic cytoplasmic granules (3 cases), lacked acinar architecture (2 cases) and/or had significant nuclear pleomorphism (1 case). In all cases, reevaluation at the time of consultation with an extended immunohistochemical panel revealed the expression of pancreatic exocrine enzymes (trypsin, chymotrypsin, and/or BCL10), confirming acinar differentiation. All six patients with follow-up died within 1 to 24 months of initial diagnosis. In conclusion, metastatic ACC should be considered in differential diagnosis of the more common liver metastasis of pancreatic origin, NET and PDAC. ACC metastatic to the liver may lack hallmark morphologic features such as acinar architecture, prominent nucleoli and eosinophilic cytoplasmic granules and may express neuroendocrine markers (mimicking NET) or exhibit prominent fibrotic stroma and pleomorphism (mimicking PDAC). Immunolabeling for acinar cell markers is warranted for cases of purported PDAC/NET with atypical features, such as purported NET with high Ki67 index and/or only patchy neuroendocrine marker labeling, or purported PDAC with luminal proteinaceous secretions rather than mucin and/or minimal pleomorphism. - Source: PubMed

Chen TianeHruban Ralph HArgani Pedram - Acinar cell carcinomas (ACCs) of the pancreas are rare neoplasms composed of cells showing acinar cell differentiation demonstrated by the immunoreactivity for acinar cells markers including trypsin, chymotrypsin, and BCL10. The differential diagnosis with more common pancreatic neuroendocrine tumors (PanNETs) may be difficult due to some overlapping morphological features and the expression of general neuroendocrine markers (synaptophysin and chromogranin A) by acinar cells, a phenomenon observed in about 35% of cases. These tumors showing a dual acinar and neuroendocrine differentiation need to be separated from PanNETs since their clinical management and prognosis are different. Little is known about the possible diagnostic role of INSM1, a highly specific and sensitive "second generation" neuroendocrine marker recently identified. With this aim we investigated INSM1 expression in 58 ACCs that, based on the expression of synaptophysin and chromogranin A, were separated into 36 pure ACCs lacking any neuroendocrine marker expression, 21 ACCs with divergent acinar and neuroendocrine differentiation, and one mixed neuroendocrine/non-neuroendocrine neoplasm (MiNEN) consisting of two separate acinar and neuroendocrine components. INSM1 expression overlapped that of synaptophysin and chromogranin A, although in several cases the number of INSM1 positive cells was less than that of chromogranin A. In conclusion, our results show that INSM1 expression in a pancreatic neoplasm should be carefully considered and interpreted in conjunction with morphology and a comprehensive immunohistochemical panel and does not give any diagnostic advantage respect to "traditional" neuroendocrine markers. - Source: PubMed

Existing deep learning methods often suffer from feature submersion when processing multi-source heterogeneous data and lack the capability for system-level topological causal inference. To address these issues, we propose a multisystem coupling modeling and anomaly traceability method based on a decoupled graph attention network (ST-DBGAE). Independent local spatiotemporal feature alignment modules are constructed to map heterogeneous sensory data into a unified latent space. This eliminates dimensional discrepancies while strictly maintaining the feature independence of underlying hardware subsystems, such as optical and gas circuits. A dynamic graph attention mechanism with sparse priors is subsequently introduced to adaptively capture time-varying coupling weights triggered by implicit interactions (e.g., thermal fluids), bypassing the need for predefined rigid physical connections. Furthermore, a dual-branch two-stage decoupled optimization architecture is designed. By blocking the cross-interference of global backpropagation, this architecture outputs a continuous equipment health index (HI) based on reconstruction errors and employs a topological difference matrix inference mechanism to reversely anchor the root-cause nodes responsible for cross-system cascading degradation. Experimental results based on over 310,000 real operational monitoring records from industrial SLM equipment demonstrate that the proposed model achieves a comprehensive diagnostic Macro-F1 score of 96.5% across eight operating states. The single-class detection rates (ACCs) of specific underlying anomalies are significantly improved. Aerobic fitness is the main determinant of match-related physical performance decline in professional football players (part 2). J Strength Cond Res XX(X): 000-000, 2026-High-intensity efforts are critical in football performance and declines in running distance and acceleration (ACC) and deceleration (DEC) capacity are often observed after the 60th or 75th minute, partly indicating player fatigue. This study investigated the decline in physical performance during a professional football match and its relationship with physical fitness parameters. Global navigation satellite system data from an entire Swiss first-division season (30 matches) were analyzed for 19 professional football players. The players' speed at the first and second lactate thresholds (sLT 1 /sLT 2 ) and peak incremental speed (PIS) were assessed using a maximal incremental treadmill test. The maximal sprint speed (MSS) was determined from match data to calculate the anaerobic speed reserve (ASR = MSS - PIS). The distance covered in low-/high-/very high-speed running (LSR/HSR/VHSR) and sprinting using individual thresholds (30% ASR), and the number of ACCs/DECs performed in 4 intensity zones (ACC 1/2/3/4 /DEC 1/2/3/4 : ±0.5-1.0, 1.0-2.0, 2.0-3.0, >3 m·s -2 ) were measured across 6 periods (P1-P6) of 15 minutes of matches. The vertical peak power was measured using squat and countermovement jumps. Significant ( p ≤ 0.05) declines (4-27%) were observed after the 60th minute across all running, ACC/DEC intensity zones, and in total distance (TD) and total number of ACCs/DECs (ACC TOT /DEC TOT ) counts, except for sprint distance and ACC 1 /DEC 1 . Moderate-to-large correlations were found between aerobic fitness (sLTs and PIS) and decreases in TD, ACC TOT /DEC TOT , and high-intensity running, ACCs/DECs between P1 (0-15 minutes) and P5 (60-75 minutes) ( r = 0.46-0.67; p ≤ 0.049). Coaches should focus on enhancing players' aerobic fitness to improve their ability to maintain physical performance throughout match-play. - Source: PubMed
