- Palmoplantar pustulosis (PPP) is a chronic inflammatory and often painful disease characterized by sterile pustules on the palms and soles, significantly impairing quality of life. Women are more frequently affected than men, and smoking is a major trigger. Under biologic therapies, especially TNF antagonists, a paradoxical PPP may occur. PPP is associated with psoriasis vulgaris and may be accompanied by osteoarticular involvement. Pathogenetically, PPP likely begins around the acrosyringium, with the pustules consisting almost exclusively of infiltrating neutrophilic granulocytes attracted by chemotactic factors secreted by activated keratinocytes. Inflammation is sustained through a self-amplifying cytokine network, including interleukin (IL)-17, IL-19, and related mediators. Treatment options for PPP include topical treatments, UV-phototherapies - particularly topical PUVA (Psoralen plus UVA) therapy- and systemic therapies. Systemic agents comprise conventional treatments such as acitretin, methotrexate, fumaric acid esters, and ciclosporin, newer small molecules like apremilast and Janus kinase inhibitors, as well as biologics. Conventional systemic therapies are often not sufficiently effective in PPP and associated with side effects. Currently, among systemic therapies, only acitretin is approved for PPP. In recent years, placebo-controlled studies have demonstrated a significant effect of apremilast, brodalumab, guselkumab and risankizumab on PPP, and further studies with topical and systemic Janus kinase inhibitors as well as IL-17A/F inhibitors are underway. - Source: PubMed

Although the characteristics and functions of IL-10 family members in mammals have been elucidated, those of IL-19L in fish remain poorly understood. In this study, we cloned the IL-19L gene from the large yellow croaker (Larimichthys crocea), designated as LcIL-19L. The open reading frame (ORF) of LcIL-19L comprises 684 base pairs, encoding 227 amino acids, and lacks a signal peptide. It possesses a typical IL-10 domain and six conserved cysteine residues. The predicted three-dimensional structure of LcIL-19L consists of eight α-helices and two β-sheets. Genomic synteny analysis revealed that it is adjacent to the IL-10 gene. LcIL-19L was constitutively expressed in all examined tissues and cells. In vivo experiments demonstrated that poly(I:C) stimulation and Vibrio alginolyticus infection induced the expression of LcIL-19L in the head kidney and spleen. In vitro experiments also showed that poly(I:C), lipopolysaccharide (LPS), phytohemagglutinin (PHA), and phorbol myristate acetate (PMA) promoted the expression of LcIL-19L in primary head kidney leukocytes (HKLs). The recombinant LcIL-19L (rLcIL-19L) protein was able to induce the expression of cytokines (IL-1β, IL-6, IL-8, IL-10, TNF-α1, and TGF-β1) and reduce the levels of reactive oxygen species (ROS). To our knowledge, this study is the first report on the cloning, expression, and functional characterization of fish IL-19L. In summary, LcIL-19L in fish exhibits immune-regulatory functions and may serve as a key molecule that regulates the inflammatory response, laying the foundation for further investigating the mechanisms underlying the immune-regulatory role of fish IL-19L. - Source: PubMed

Wei ZuyunAi ChunxiangChen Xinhua - Head and neck squamous cell carcinoma (HNSCC) is a highly heterogeneous malignancy characterized by altered lactate metabolism, where traditional prognostic indicators are insufficient for precision medicine. This study aimed to construct an enhanced CT radiomics model integrated with lactate metabolism gene-related (LMGR) genomic signatures for HNSCC prognosis using TCGA and TCIA databases. A cohort of 399 HNSCC patients was analyzed. Analysis of 204 lactate-related genes identified 24 differentially expressed LMGR genes (DELMGR). Univariate Cox regression revealed that among these, PKLR, IL19, and CXCL9 exhibited protective effects (HR = 0.932, 0.885, and 0.931, respectively). A lactate classification score (LCS) was derived from the analysis of these three genes, demonstrating a significant correlation with overall survival (OS) in both univariate (HR = 1.807, 95 % CI: 1.346-2.424, P < 0.001) and multivariate assessments (HR = 1.772, 95 % CI: 1.296-2.424, P < 0.001). From enhanced CT images, 2060 radiomic features were extracted. Subsequently, after feature selection using mRMR and RFE algorithms, a support vector machine (SVM) model was built to predict LCS, which generated a radiomics score (RS). The model demonstrated AUC values of 0.773 and 0.760 in the training and validation datasets, respectively. The RS distribution significantly differed between lactate subtypes in the training cohort (P < 0.001), with specifically higher RS in the high-risk LCS group. High RS was associated with poor OS (HR = 3.582, 95 % CI: 1.240-10.348, P = 0.018) and was correlated with clinical features such as the perineural invasion and the margin status. Mechanistic analysis indicated that the high RS group was enriched in an immunosuppressive microenvironment and was associated with fatty acid metabolism pathways. This enhanced CT-based radiomics model effectively predicts lactate-based stratification, demonstrating potential prognostic value in HNSCC and providing novel biomarkers as well as a non-invasive predictive tool for prognostic assessment. - Source: PubMed
