- : Ceftolozane/tazobactam is a β-lactam/β-lactamase inhibitor with potent activity against multidrug-resistant Gram-negative pathogens, notably . Its use is vital in the management of severe infections in ICU patients, especially where resistance limits first-line antibiotic options. This study aimed to describe the clinical characteristics, infection profiles, antimicrobial therapies, and hospital outcomes of ICU patients in Brazil who were treated with ceftolozane/tazobactam. : A multicenter retrospective analysis was conducted on ICU patients treated with ceftolozane/tazobactam across twelve private hospitals in Brazil admitted between July 2018 and February 2023. Data were extracted from electronic medical records, including demographics, comorbidities, infection characteristics, antimicrobial usage, and hospital outcomes. Additionally, microbiological data and treatment details were evaluated. Descriptive statistics were used for analysis. : The study included 104 patients, with a median age of 78 years (IQR 61, 87) and 43% of whom were female. Pneumonia (57%) and urinary tract infections (19%) were the primary indications for treatment. Mechanical ventilation was required in 62 patients (60%). Pathogens were isolated in 44 cases, with ESBL-producing Gram-negative bacteria (34%) and carbapenem-resistant strains (18%) being the most common. Empirical therapy was initiated in 57% of cases, with a median treatment duration of 10 days. Concomitant antibiotics were used in 64% of patients. Mechanically ventilated patients exhibited higher mortality (66% vs. 29%) and prolonged ICU stays (57 vs. 20 days) as compared to non-ventilated patients. : Ceftolozane/tazobactam demonstrates efficacy and safety in managing multidrug-resistant Gram-negative infections in ICU settings. However, its empirical use, driven by high resistance rates, underscores the importance of microbiology-guided therapy. The observed high mortality and prolonged ICU stay highlight the critical need for optimized infection management strategies and continued antimicrobial stewardship in critically ill populations. - Source: PubMed

The quality and validity of DCEs largely depend on the formative phase of attribute development. This paper documents a systematic attribute development process for a DCE assessing preferences for long-acting HIV prevention products among pregnant and breastfeeding women (PBFW) in Kenya. We applied the Helter and Boehler four-stage attribute development framework, comprising raw data collection, data reduction, removal of inappropriate attributes, and appropriate wording. Through literature review and qualitative approaches, we identified twelve attributes for products formulated as oral tablets, injections, vaginal rings, and implants. Six of the twelve attributes were considered plausible, salient, and capable of being traded, namely dosing frequency, effectiveness in HIV prevention, ability to prevent pregnancy, ability to prevent sexually transmitted infections, product access location, and product cost. A pilot among thirty PBFW confirmed the clarity and relevance of the six attributes, informing their inclusion in the DCE. Documenting this transparent step-by-step process improves transparency and facilitates objective evaluation of the DCE's formative phase. These findings contribute to methodological rigour in DCE design and provide empirical evidence for expanding preference-based HIV prevention strategies that will benefit PBFW. - Source: PubMed

Ngugi Sophia MEchoka ElizabethWere VincentThuo NicholasOgello ValleryGikwa MaryeveMuteti NelsonMwangi PaulKazungu JacobNgure Kenneth - We describe a case of a previously healthy 19-month-old female who experienced rapid neurological deterioration following infection with influenza A (H1N1) and was found to have neuroimaging features characteristic of acute necrotizing encephalitis (ANE), including bilateral thalamic involvement and additional multifocal, symmetric gray- and white-matter lesions. At initial presentation, multiple proinflammatory cytokines were markedly elevated in the peripheral blood. Interestingly, while multiple cytokines were also elevated in cerebrospinal fluid (CSF), interleukin-6 (IL-6) and IL-8 levels were substantially higher in CSF (590 and 1330 pg/ml, respectively) than in serum (118 and 68.2 pg/ml, respectively), providing insights into disease pathogenesis and central nervous system-specific immune activation. The patient was treated in a stepwise manner with multimodal immunomodulatory therapy, including intravenous immunoglobulin (days 1-2), corticosteroids (start day 1), IL-1 receptor antagonist anakinra (start day 2), and plasma exchange (start day 3). Blood cytokine levels declined rapidly with initiation of immunomodulatory therapy. She demonstrated a favorable clinical response with complete neurological recovery, despite an unfavorable ANE severity score of 6 that has historically been associated with high mortality. This case highlights the importance of early recognition of ANE in young children with recent influenza infection, the need for prompt multidisciplinary management, consideration of targeted immunomodulatory therapy, and further investigation of cytokine profiling as a tool to improve diagnosis and guide treatment. - Source: PubMed
