- Abdominal aortic aneurysms (AAAs) are a life-threatening cardiovascular disease for which there is a lack of therapy preventing aortic rupture. During AAA formation, pathological vascular remodeling is driven by an imbalance of proinflammatory and anti-inflammatory macrophage responses, and mechanisms regulating macrophage-mediated inflammation remain undefined. We report a case of multiple giant aortoiliac aneurysms comprising a ruptured giant AAA and bilateral giant common iliac artery aneurysms (CIAAs). A 59-year-old male was transferred to our emergency department with a ruptured AAA. Computed tomography angiography (CTA) showed a ruptured giant infrarenal AAA measuring 16.9×9.7 cm and bilateral CIAAs measuring 9.6×9.7 cm and 9.0×8.7 cm. The patient underwent successful open surgical repair with an aortoiliac bifurcated graft. An intraoperative left ureteral injury was immediately repaired by end-to-end anastomosis over a double-J stent. Postoperative CTA demonstrated successful graft reconstruction without leakage. The patient recovered without further major complications and was discharged on postoperative day 23. This case highlights the technical challenges of treating multiple giant aortoiliac aneurysms and demonstrates that emergency open repair can achieve a favorable outcome. - Source: PubMed

This study examines organizational characteristics of AAAs associated with their shift to contracting with health care organizations. The study analyzed data using repeated waves of the National Survey of AAAs and the Community-Based Organization-Health Care Contracting Survey. Logistic regression assessed how organizational characteristics are associated with contract formation. Findings reveal that shifting into formal contracting was more likely for independent non-profit AAAs, those AAAs that provided care transition services, and AAAs with high annual budgets. Higher proportion of funding received from the Older Americans Act was negatively associated with a change in contracting status. This study suggests that organizational autonomy, financial stability, and diverse funding streams are associated with a higher capacity for entering into health care contracts. - Source: PubMed
