- Lipoid proteinosis (LP), or Urbach-Wiethe disease, is an ultra-rare autosomal-recessive disorder caused by loss-of-function variants in the ECM1 gene. It is characterized by the progressive deposition of hyaline-like material in the skin, mucosae, and central nervous system. This series describes three siblings (aged 16, 24, and 25) from Gujranwala, Pakistan, born to non-consanguineous parents. All patients presented with infant-onset hoarseness of voice and recurrent vesiculobullous eruptions that evolved into waxy papules and hyperkeratotic plaques. Physical examination revealed the pathognomonic "beaded" eyelid papules (moniliform blepharosis), pock-like facial scarring, and a thickened lingual frenulum restricting tongue protrusion. Despite shared genetics, intrafamilial variability was observed, with the male sibling exhibiting a more severe "leonine-like" facies. Neuropsychiatric symptoms, including aggression and insomnia, were noted across the series. Due to resource constraints, the diagnosis was established clinically. Management focused on symptomatic control using oral acitretin (0.5 mg/kg/day), antihistamines, and intensive topical emollients. At 12-week follow-up, patients showed significant reduction in pruritus and stabilization of skin lesions, though hoarseness and established scarring remained unchanged. This series underscores the importance of recognizing cardinal clinical triads-moniliform blepharosis, mucosal tethering, and early hoarseness-to diagnose LP in resource-limited settings where genetic or histological confirmation is unavailable. - Source: PubMed

Extracellular matrix 1 (ECM1) is a glycoprotein with distinct biological activities ranging from immunological to physiological roles. The impact of calyx extract on ECM1 in ovarian cancer has not been thoroughly investigated, particularly in cell proliferation inhibition and apoptosis. MTT assay was performed to determine anticancer activity in Rosella extract. The cell and nuclear morphology were examined along with colony formation, and migration assay. The effect of the extract on the molecules involved in the cell signaling mechanism was evaluated using western blotting and PCR. Calyx extract cytotoxicity presented by the MTT assay at a 2.8 mg IC50 value, with atypical nuclear morphology and reduced cell migration. Rosella crude extract influences ECM1 glycoprotein, AKT1, and Cyclin D1 suppressing colony-forming ability. Apoptosis gets triggered, resulting in upregulation of different caspases, BID, and suppression of Bcl-2. The Rosella crude extract inhibited cell growth in OVCAR3 cells regulating the ECM1/AKT1/Cyclin D1 pathway, suggesting a potential approach for treating ovarian cancer. - Source: PubMed

Pande KaumudiKannan Anbarasu - Multisite chronic pain (MCP) is a complex and increasingly prevalent health issue that significantly impairs quality of life. This study aims to identify potential therapeutic targets for MCP through a proteome-wide Mendelian randomization (MR) approach. We conducted a proteome-wide MR to explore the causal associations of plasma proteins with MCP. MCP used data from a genome-wide association study including 387,649 samples. We used protein data from UKB-PPP including 54,219 samples as the discovery analysis, and from Finngen as the replication analysis. Multiple follow-up analyses were used to investigate the potential function of the candidate proteins. Finally, druggability evaluation and phenome-wide MR analysis were used to assess the priority of these targets. We identified 11 plasma proteins significantly associated with MCP. Increased levels of LRP11, BCHE, DAG1, and SUOX exhibited protective effects, while LATS1, CEP170, SLC27A4, HEXIM1, ECM1, C8B, and MST1 increased MCP risk. After multiple validations, ECM1, C8B, LRP11, BCHE has the strongest convincing evidence. Besides, mediation analysis found 4 reliable combinations, revealed the role of plasma proteins in traits influencing MCP. Finally, druggability evaluation and phenome-wide MR indicated that C8B, and BCHE had the highest priority. - Source: PubMed
